About the clinic

The frequency of cardiovascular diseases is steadily increasing every year. However, not always a patient can get adequate help due to insufficiently high level of technology and qualification of doctors. German medicine today is one of the best, which is confirmed by various international experts. Cardiologist Dr. Sigmund Silber has extensive experience in treating patients with various cardiovascular diseases. Dr. Silber began his training in 1968 at the Ludwig Maximilian University Medical School. After only two years of training, Dr. Silber was already hired as a researcher at the Institute of Experimental Pharmacology at the University of Munich. Working for several years and receiving higher education, Sigmund Silber conducted the necessary research to write and defend his dissertation on the topic: «The effect of Aescin on blood circulation». In 1975, Dr. Silber officially began his medical practice.

During the next few years, Dr. Silber did his residency and postgraduate training at the German Heart Center in Munich under the supervision of Prof. W. Rudolph, MD. In 1982, Dr. Silber was confirmed as an assistant professor at the Center for Cardiology Clinic of the Ludwig Maximilian University. In the same year, he was appointed head of the Department of Nuclear Cardiology. Due to his hard work, writing various scientific papers and participating in international conferences, Dr. Sigmund Silber was selected as a member of the Isotope and Formulary Committee of the University of Munich.

In addition, working in the field of minimally invasive techniques and conservative drug treatment, he headed the laboratory for ST-segment Holter monitoring at the University of Munich. Thanks to the experience gained and patients treated, Dr. Silber defended his doctoral thesis in 1986 at the Ludwig Maximilian University under the supervision of Professor E. Buchborn. The work of the cardiologist is devoted to the peculiarities of the action of nitrates. In 1987, after defending his doctoral dissertation, he went to the USA for an internship, where he worked for several years as an assistant professor at the Department of Cardiology under the supervision of G.M. Pohost.

Since 1990, Sigmund Silber started working in several outpatient clinics as well as in a catheterization laboratory. In addition, he became a full professor of medicine at the Ludwig Maximilian University in 1998. Starting in the 1990s, Sigmund Silber developed the Isar Heart Center. Today, this institution is one of the leading medical cardiology centers in Munich and Germany.

The institution provides a wide range of diagnostics and treatment of heart and vascular diseases. The success of heart disease treatment depends to a large extent on the accuracy of the diagnosis, so treatment at the clinic of Professor Dr. Silber begins with an examination using high-tech equipment and methods that provide reliable data on the state of the heart and blood vessels, and allow to make an accurate diagnosis. Each patient undergoes an ECG in 12 leads, at rest, during exercise or, if necessary, long-term monitoring of the heart.

Catheterization or coronary angiography is used to diagnose more complex rhythm or conduction disorders. This procedure can be performed as an outpatient or inpatient procedure. In addition, coronarography allows not only to exclude or confirm any heart disease, but also to monitor the effectiveness of drug therapy. Based on the results of the study, the doctor can adjust the dose of medications. During coronarography, the doctor can examine the vessels of the heart, such as the left and right coronary arteries.

In addition to coronarography, the clinic performs ventriculography. With the help of this study, the left ventricle of the heart is visualized to exclude various pathologies. Aortic pathology occupies about 10% of all heart diseases. In order to exclude any pathology of the aorta, aortography is performed. The above techniques are diagnostic measures.

A large proportion of heart diseases are rhythm disorders. Very often such disorders can be fatal, so adequate therapy plays a key role. At Prof. Silber’s clinic, the effective treatment of rhythm disorders is given special attention.

In rhythm disorders, the heart begins to contract irregularly, which leads to disorganization of the entire cardiovascular system. This can eventually lead to cardiac arrest and death. To avoid this, various systems are installed in the heart to ensure regular heartbeat.

Installation of cardioverter defibrillators, pacemaker systems and pacemakers. Using a small catheter, a small-sized probe is introduced into the heart cavity and placed directly in the area of the atrium of the heart. Then the doctor adjusts the necessary mode of operation on the device and after forming a bed on the anterior chest wall implants the device. Thus, if there is a rhythm disturbance in the heart, the device receives information from the probe and gives the necessary impulse to synchronize the contractions.

Implantation of an event recorder. Not always conventional diagnostic methods can help in making the correct diagnosis, as arrhythmias can occur at absolutely any time. In order to determine the type of arrhythmia and whether there is a risk of arrhythmia, an event recorder is implanted in the patient. It reads the heart function over time and can also perform emergency defibrillation if necessary.

In addition to irregular heart rhythms, a frequent cause of cardiovascular disease is the presence of plaque in the blood vessels or their narrowing. A narrowed vessel lumen prevents blood from adequately pushing blood to organs and tissues. To remove this mechanical obstacle, a number of surgical procedures are performed at the Isar Heart Center. The peculiarity of these interventions is mini-invasiveness, the duration of the operations is about 1 hour. The post-operative period is short and this intervention can also be performed on an outpatient basis. 

Percutaneous coronary intervention. Using a catheter, a stent is placed in the lumen of the vessel above the site of the block, which prevents the vessel from narrowing back, after which the device is removed and the vessel remains of the required diameter. Thus, blood flow is restored.

Percutaneous transluminal coronary angioplasty (PTCA). This is a modification of the previous method. With it, a stent is placed in the vessel, and it can be coated with various drugs to dissolve recurrent clots.

Coronary stenting. This procedure is usually performed after coronary angiography. It reveals the number of affected vessels, localization of plaques. This information allows to understand how many vessels require stenting.

Carotid artery stenting. Given that atherosclerosis affects all vessels in the body, a plaque can form in the carotid artery. This is dangerous because the plaque can block this vessel and cause a stroke. Therefore, stenting helps to remove this lesion and thereby reduce the risk of developing serious complications.
