Coronary angiography

Coronary angiography is an invasive radiologic study of the blood vessels of the heart. The vessels that deliver oxygenated blood to the myocardium (heart muscle) are called coronary arteries. Coronary angiography is performed for both diagnostic and therapeutic purposes — stent implantation. It’s the «gold standard» for treatment coronary heart disease.

In what cases is coronary angiography performed?

Coronary angiography is performed to confirm the severity of the disease and the presence of other important comorbidities.

  • In coronary atherosclerosis, there is thickening and narrowing of the elastic walls of the arteries due to plaque formation.
  • When there is spasm (contraction) of the coronary vessels.
  • In angina pectoris, where drug treatment is ineffective and symptoms of the disease become more frequent.
  • При острIn acute myocardial infarction (acute occlusion of a coronary vessel by a thrombus) in order to restore arterial patency and perform myocardial perfusion.
  • In unstable angina with severe arrhythmias — significantly reduces mortality and the risk of recurrent myocardial infarction.
  • Before and after coronary bypass placement.
  • In congenital coronary anomalies.
  • In the evaluation of valve defects.

Is prior preparation necessary?

Before coronary angiography is performed, your doctor must get your consent for the test. You will be carefully evaluated for x-ray (iodine-containing) allergies . In case of any type of allergy, it is recommended that you take glucocorticoids, antihistamines and antacids 18-24 hours before the procedure. You should not eat breakfast on the day of the procedure . The area where the piercing is to be performed is wiped and disinfected.

Method of conduct 

Coronary angiography involves the following steps:

  • The puncture is performed with a special needle in the area of the wrist (radial artery) or the artery of the foot in the area of the inguinal crease (femoral artery).
  • A small flexible tube (dessile) is inserted into the vessel.
  • Through it, another longer tube (catheter) along the circulatory system travels through a metal guide to one of the two heart (coronary) arteries.
  • Contrast is injected sequentially into the left and then the right coronary artery, and arterial contrast is observed while a series of digital x-rays are taken.
  • The study also includes the use of a left ventriculograph — injection of contrast into the left ventricle of the heart to assess the volume of the heart cavity and mitral valve function.
  • По After completion of the study, a compression dressing is made at the puncture site, which is removed after 2-4 hours for arm access and 6-8 hours for leg access.
